Healthy Families America/Community Health Worker Supervisor–Nurses for Newborns

Full Time - Benefit Eligible Position

Position Description:

NFN is seeking an HFA/CHW Supervisor who will supervise 2.5 FSS positions and also will carry a small caseload of clients to provide services under our Healthy Families America affiliation.

Qualifications:

  • Primary duties include:
  • Supervision of Family Support Specialists to include training and orientation and coordination of outreach efforts. Reviews FSS documentation.
  • Provides direct support and monitors numbers of cases assigned to FFS staff.
  • Independently, within the parameters of established protocols visits clients in their homes, or virtually when appropriate, serving as an advocate and assisting the client and family with locating resources, obtaining regular medical care, and meeting transportation needs.
  • Follows up on care plan goals, educating on home safety, promoting bonding between caregivers and child, and recognizing and preventing child abuse and neglect.
  • Represents the agency in community events and by recruiting new clients.
  • The ideal candidate will possess a master’s degree in human services or fields related to working with children and families, or a bachelor’s degree in these fields with 3 years of relevant experience, or high school/GED minimum requirement but with commensurate HFA experience.
  • Demonstrated knowledge of and experience with community issues and resources.
  • Home visiting background and strong experience in early childhood preventative services
  • Excellent customer service skills, both internal and external. Effective verbal and written communication skills. Excellent computer skills, including data entry skills.
  • Ability to establish a trusting and therapeutic relationship with clients, caregivers and families.
