Intake Coordinator – BSW or CHW-C Certification

Position Description:

Do you want to be part of a team that works to ensure babies and their families are healthy, safe, and supported? Do you have exceptional customer service and organizational skills? We are hiring a talented individual as an Intake Coordinator and you might be the one!

Primary duties include:
Reviews referrals to determine program eligibility, contacts referrers if additional information is needed.
Returns all voicemails within 24 hours
Contacts and works with clients who have been referred for services. Obtains demographic and medical/social factor information and data
Enters referrals into client database within 24 hours
Provides resources to clients in need and shares new resources with Home Visiting Staff
Processes intake closing and home visitor closing letters and maintains log of these letters
Enters address changes and reports to CNO if address change results in a change of authorization
Partners with major referrers to disseminate updated information regarding referring clients to Nurses for Newborns
Other activities and duties as assigned

Qualifications:

The ideal candidate will possess either a BSW or CHW-C certification or equivalent.
and 2-3 years experience in a non-profit or medical office setting
Excellent customer service skills, both internal and external
Effective verbal and written communication skills
Excellent organizational skills, strong attention to detail and ability to meet deadlines
Excellent computer skills, including data entry skills. Ability to multi-task, maintain confidentiality and be flexible in a dynamic work environment
This is a full-time in-office benefit eligible position.
