Position Description:

NFN nurses independently provide in-home nursing visits to pre-natal, postpartum and newborn clients, following Clinical Guidelines in planning care, assessing client needs and in determining and utilizing screening tools. The Staff RN documents their work by entering assessments, screenings, care plans and referrals in our electronic database following NFN guidelines. Nurses work with hospitals, physicians, managed care and community agencies to provide care for clients and refer clients to community agencies and other resources as appropriate. Locations within MO to be determined and may vary depending upon the needs of NFN.

Position Description:

Nurses for Newborns is seeking the right individual who can independently and in collaboration with a Registered Nurse and/or Community Health Worker, provide comprehensive in-home clinical social work services to prenatal, post-partum and newborn clients and their families. The primary focus of this position is in-home (virtual) counseling. You will be following the NASW Code of Ethics in planning care, assessing client needs, providing therapy and in determining and utilizing screening tools. You will also connect clients with appropriate community resources and actively maintain relationships with these agencies in order to enhance collaborative efforts that benefit NFN clients. You will be working with hospitals, clinics, physician offices and community agencies to provide comprehensive in-home services for clients.

NFN is seeking a Community Health Worker who will provide services under our Healthy Families America affiliation. Primary duties include: Visits clients primarily in their homes. Some visits may be virtual as appropriate. Closely collaborates with the Staff Nurse for provision of services and referrals. A knowledge of community resources is preferable. Assists with cultural mediation among individuals, communities, health and social service systems. Provides culturally appropriate health education and information. Provides case management and system navigation. Provides coaching and social support. Advocates for individuals and communities. Assists with building individual and community capacity. Conducts community outreach. Completes documentation in clinical database for client contacts, scheduled appointments and visits based on NFN policies.
